Manual therapy is a hands-on treatment approach where a physiotherapist uses their hands — rather than machines or exercise alone — to diagnose and treat pain, stiffness, and restricted movement in joints and soft tissue.
What techniques does it include?
Manual therapy covers several specific techniques: joint mobilization and manipulation (small, controlled movements to restore joint motion), soft tissue release (working on muscles and fascia to reduce tightness), myofascial release, and muscle energy techniques (using controlled muscle contractions to improve mobility). A physiotherapist selects techniques based on what your assessment shows.
What does it help with?
Manual therapy is commonly used for back and neck pain, joint stiffness, muscle tightness and spasm, and as part of recovery from sports injuries or surgery. It's rarely used alone — most treatment plans pair manual therapy with targeted exercise so the improvement in mobility is maintained, not just felt in the moment.
Is it the same as a massage?
No. A massage is generally relaxation- or general-wellness-focused. Manual therapy is a clinical assessment-driven treatment — a physiotherapist identifies exactly which joint or tissue is restricted and why, then applies a specific technique to address that finding, as one part of a broader treatment plan.
Does it hurt?
Some techniques can cause brief discomfort, particularly when working on a genuinely restricted joint or tight muscle, but a competent physiotherapist works within your tolerance and explains what to expect before starting. Ongoing sharp pain during treatment is not normal — tell your physiotherapist immediately if that happens.
