Back pain covers a wide range of experiences, from a dull ache after a long day at a desk to sharp pain that limits how you move. Most of the back pain physiotherapists see is mechanical — meaning it comes from how the muscles, joints, and discs of the spine are moving and loading, rather than from a disease process. It can build up gradually from posture and habits, or start suddenly after lifting, twisting, or an awkward movement.
This page describes back pain in general terms and how physiotherapy generally helps with it. It is not a substitute for an assessment, and it does not tell you what is causing your specific pain — that is what your first appointment is for.
Long periods sitting, especially with poor desk setup, can leave the lower back and supporting muscles stiff and strained.
Lifting awkwardly, sudden twisting, or unaccustomed physical activity can strain the muscles and ligaments supporting the spine.
Changes in the discs between the vertebrae can contribute to stiffness and localized pain, sometimes with pain that spreads toward the hip or leg.
Both a sedentary routine and a sudden jump in activity — a heavy gardening weekend, a house move — are common triggers.
A physiotherapist starts by assessing how your spine moves, which movements are restricted or provoke pain, and how your surrounding muscles are working. From there, treatment typically combines hands-on manual therapy — joint mobilization and soft tissue work — with an exercise plan built around what the assessment found, and practical advice on posture, movement habits, and pacing activity.
The plan is reviewed and adjusted as you progress. This page describes the general approach, not a fixed program — what suits your back depends on your assessment.
Your first session starts with questions — how the pain started, what makes it better or worse, and how it is affecting your daily activities. Your physiotherapist then assesses your spine's movement and your general strength and flexibility. Together, you will discuss what was found and agree on a plan, including realistic first steps rather than a rigid prescription.
Most back pain is manageable with assessment and physiotherapy, but a small number of cases need urgent medical attention first. Go to a hospital emergency department or call emergency services if your back pain comes with any of the following:
This list is general safety guidance, not a way to self-diagnose. If in doubt, seek medical assessment rather than waiting.
Back pain that spreads down through the buttock and leg is often described as sciatica. If that sounds like what you are experiencing, our sciatica physiotherapy page covers it in more detail.
Tell us how it started and how long it has been going on. We will book the right assessment slot — at the clinic or at your home.
